Skin Care Ingredients to Avoid While Nursing

While I’m sure you know by now that you need to change your skin care routine while pregnant, but how does this translate over to breasting feeding? Are there skin care ingredients to avoid while nursing? Do not use this post to ask about nursing safety. Nursing safety of specific products is address in my pregnancy safety products lists, I delete all questions here about pregnancy safety.

Skin Care Ingredients to Avoid While Nursing

Luckily, it turns out that breast feeding is very different than being pregnant. The placenta is basically a big sieve, it filters out a lot of stuff but since it is non-specific it does allow a lot of stuff to cross over. A lot of the ingredients to avoid while pregnant are because of the timing of the exposure, your baby was busy making organs!

When nursing, the body is pretty specific about what goes into breast milk. It isn’t filtering everything into the breast milk, instead things are much more specific. The timing of exposure (and potential side effects) is just very different once the baby is out! Those organs have been made, and the baby is also just a lot bigger. So, the list of skin care ingredients to avoid while nursing is much shorter than the list of ingredients to avoid while pregnant.

Medications and Nursing

One thing you need to be worried about are your medications. Nursing safety is a bit different than pregnancy safety for medications. Everything should be looked up in a lactation safety book (Medications and Mother’s Milk or Drugs in Pregnancy and Lactation) and discussed with your physician. L1-L3 are considered safe, L4 and above is not safe.

Skin Care Ingredients to Avoid While Nursing

Retin A/Accutane, Retinoids, Vitamin A: Used topically, Retinoids are a category L3, but orally (as with Accutane) they fall to L4. So, topical is ok, systemic is not. Having said that, over the last 4-5 years there’s been a big trend toward recommending avoidance of retinoids while nursing. I’ve heard this from OB, Derm and Peds. I am very conservative in my recommendations and I’m currently recommending avoidance and I screen it out in my product reviews. I am also currently screening Bakuchiol as a retinoid as there is no evidence regarding safety.

Hydroquinone and other skin lighteners: While there is no data on Hydroquinone in humans, it is absorbed and quite water soluble, and based on some pharmacodynamics (the pKa, which is too complicated to get into here as it was a many hour discussion when I was in medical school), it may be trapped in the breast milk if it reaches it. So, it is recommended to avoid. I would include Arbutin and Kojic Acid as ingredients to avoid.

Hemp/Cannabis: Similar to the discussion on the ingredients to avoid while pregnant page, we just don’t have enough info about the safety of hemp and cannabis while pregnant or nursing, so I currently recommend avoidance.

Pretty much everything else: Safe!

    Thanks so much for doing these posts! They are SO helpful! I’m wondering if you could offer any advice or a resource on how to read ingredient lists? For example, will glycolic acid always be listed that way? I’m currently using a cetaphil moisturizer and I don’t see anything in the ingredients listed as “glycolic acid” but I do see “butylene glycol” and “caprylyl glycol.” Are those the same as “glycolic acid”?

      There are a couple of ingredients that will be listed different ways, but I”m afraid it would be pretty difficult for me to create a post about that. When in doubt, google is your friend. Glycolic Acid is always listed that way, the “butylene glycol” and “caprylyl glycol” are different and are fine 🙂

      I’ve always read that it is safe and I don’t know of anything that says differently, but there are some dermatologists that say not to use it while nursing. I’m not sure why though? It’s a recent thing that I’ve noticed over the last year or so in my medical groups. I used that one wile nursing!

      Gylolic acid doesn’t need to be avoided while nursing, it’s much easier to avoid things while nursing than while pregnant!

          I think some thing strange reverted back to a previous version of this post (a few years ago actually), because I swear I had updated that to say to avoid it during nursing some time ago. And yes, I’ve been telling people in comments for years to avoid it. I’ve reupdated above, it should be avoided. I’d look at using a vitamin C product instead!

      At this point no, your baby is mostly getting calories and nutrition from the food anyways. Retinoids in nursing some think are fine, I haven’t heard a convincing argument that they aren’t other than “we avoid it in pregnancy”…

      Anything that is ok while pregnant is fine while nursing, Charlotte Tilbury was actually added to the big list of pregnancy safe products over the weekend. There are some products that weren’t safe while pregnant, but they were all fine while nursing.

          I consider retinol fine during nursing based upon all the reading I’ve done and people I’ve talked to (derms and OBs). However over the last year or so I’ve heard more doctors say to avoid retinol while nursing, not because of any new studies that have come out, but just as an extension of avoidance while pregnant.

      It’s fine! It’s a salt of hyaluronic acid, a HUGE molecule that is too big to penetrate much or move around in your body. It holds moisture in certain areas, you’ll find it in a lot of moisturizers. It’s naturally made by your body.

      For nursing I think it is fine. Note there have been more dermatologists/OBs who have been saying to avoid retinol in nursing but I don’t know (and can’t find) and new information to support this. They just are uneasy about it, from all the discussions I’ve had.

          Retinols are all up in the air. I’ve only in the last few years started hearing them mentioned as something to avoid while nursing, I’ve been debating about adding it on to my “avoid in nursing” list. Retinyl Palmitate is one of the over the counter, less active versions (and weaker). It’s kind of up to you how conservative you want to be with it. I used retinoids while nursing, if that helps.

      1) It isn’t an acid as you think of it, it’s an extremely large molecule also called Sodium Hyaluronate. The difference is if a hydrogen or sodium is attached to something.
      2) This molecule is so large that it can’t penetrate into the skin. There are companies that claim that they have smaller versions that can penetrate into the skin. The problem is that none of those brands ever have data showing that it actually does penetrate, they just claim it does. That always gets a side eye from me given #1.
      3) Even if it DOES penetrate into the skin, it doesn’t really matter. Why? Because your skin (and esp your dermis) already makes this molecule on its own. If it is too dangerous to apply, why is your skin also making it?? That is why there isn’t information about it. Your skin is making it. Applying it topically (where it will likely sits on the surface of this skin) shouldn’t be a problem if it is already found IN YOUR SKIN.

      It’s fine while nursing, the risks of using salicylates while pregnant and therefore the subsequent issues in baby are non-issues after delivery. This is fine while nursing, I would avoid it while pregnant due to the willow bark and the salicylic acid.

      I can’t comment on safety without a list but please read above for which ingredients you need to avoid. It’s only hydroquinone/arbutin and maybe retinol if you’re being very conservative, hydroxy acids are fine.

      I hadn’t heard of it, a quick google search says those are liposomes, it’s their proprietary name for them. Liposomes are basically little pockets that can penetrate more deeply into the skin, and companies will put different active ingredients inside of them.

I had tried researching them and couldn’t find anything except a site that talked about inflacin and how it works for rheumatoid arthritis and other inflammatory diseases and Bellame says it decreases redness and inflammatory response on face caused by sun, pollutants, etc 🤷‍♀️ So I wasn’t sure about safety with breastfeeding.       That’s all the ingredients listed on the brand’s site too. They don’t specify what type of vitamin A is in there, and if it is a retinoid some people would still avoid that while nursing. Beta-Carotene would be ok though. So, up to you!

      It’s ok for nursing, but not during pregnancy as it is highly absorbed and we think it crosses much more easily across the placenta. We don’t have hard proof that it causes any issues, but it makes everyone nervous! Those of us that are more conservative would recommend using only pregnancy safe products while nursing as well as we’re more restrictive then, but the creation of breast milk is more restrictive than the placenta so…. that’s kind of where we’re at. No hard evidence, just different degrees of worry if that makes sense!

      I can’t tell for sure where one product starts and another starts, but all of the ingredients here look ok. I can’t comment on drinking collagen tablets, there aren’t proven benefits over a diet with enough protein and each tablet is different, so may or may not be safe.

      Willow bark is ok for nursing. This has chemical sunscreens, the willow bark, a salicylate, all of which are ok for nursing but not pregnancy. There’s also retinol, definitely not safe for pregnancy, and if you’re on the conservative side people avoid that during nursing as well.

      It’s one of those up to you things. I don’t know of any studies specifically looking at it in nursing, but I know OBs who say you can use it and others that say you can’t. I personally did use it, but I’ve seen more and more OBs say not to since then. So it depends on how conservative you are.

      The retinoid is something to avoid for sure during pregnancy, but I haven’t seen any evidence that you absolutely must avoid it during nursing. There have recently been more and more OBs that suggest avoiding it during nursing as well, almost as an extension of the pregnancy avoidance. But really, the reason to avoid it while pregnant is to avoid certain birth defects, which can’t suddenly form after birth. Long story short, I wouldn’t worry about it in your case! 🙂 I hope that helps.

      There’s a lot of ongoing debate about this. When screening products I’ve been cautioning against the use of retinoids while nursing, but there isn’t strong evidence either way. Some physicians say it is fine, some say you should exercise caution.

      As well, you need to remember what L3 stands for: “L3 MODERATELY SAFE – Studies in breastfeeding have shown evidence for mild non-threatening adverse effects OR there are no studies in breastfeeding for a drug with possible adverse effects”

      The cautions during pregnancy are due to birth defects, which can’t occur/develop after birth.

      It isn’t an ingredient that has enough evidence in it for the medical community to say it needs to be avoiding in nursing or pregnancy. But if you’re that worried about it, I would just avoid it. Note you can’t suddenly develop chromosomal mutations in such a manner, it’s something that would have been determined by the genetics of the egg and sperm, not post-birth exposure.

      These aren’t full ingredient lists, but with the licorice derivatives none will be ok for pregnancy, I can’t comment on nursing.

I’m SO relieved to read I can use glycolic acid and Salicylic acid as there’s so much contradiction online about percentages…does this matter?
